This quote means if you accomplish something great and do it truthfully it's great. If you don't do it truthfully you have great pain just like Mark Mcgwire. This shows how you shouldn’t cheat or lie   because the pain gets you. It can also mean if you do something good but you did it by cheating it doesn't count mentally. A good example of this quote would be if you win a world cup, but half of your players are from different countries one of them is going to feel the guilt and tell the truth.
